The Air Armament Center Operational Contracting Office, Eglin AFB, FL, intends to solicit and award a sole source firm-fixed price contract for Acclarent Relieva Balloon Sinuplasty Devices and Associated Accessories under the authority of FAR 13.106(b)(1) to Acclarent Inc., of Menlo Park CA. Anticipated award date is 02 August 2012. This requirement is for:

CLIN 0001: Acclarent Sinus Items, QTY 1 Lot
Includes the following components: Relieva Ultra Sinus Balloon Catheter, 6 mm diameter Qty 1 box; 7 mm diameter, Qty 1 box; Sinus Guide Catheter, Qty 3 boxes; Inspira Air Balloon Dilation System, Qty 1 box.; Relieva Luna Sinus Illumination System and Accessories, Qty 2 boxes; Relieva Solo Pro Sinus Balloon Catheter, Qty 2 boxes; Relieva Sidekick Low Profile Sinus Catheter Handle, Qty 1 box; and Relieva Stratus MicroFlow Spacer, Qty 2 boxes.

These items must be delivered to: 96th Medical Material Management Squadron - FM2823 EGLIN AFB, FL 32542 within forty eight hours after receipt of order on an FOB Destination basis. The North American Industry Classification System (NAICS) for this acquisition is 339112, small business size standard: 500 employees. This notice is not a request for competitive proposals/quotes; however, all responsible sources may submit a bid/proposal or quotation. The government will consider all responses received within ten (10) days of publication.
