This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in FAR Subpart 12.6 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for VA248-15-1620, issued as a Request for Quote (RFQ), quotes are being requested and a written solicitation will not be issued. This notice and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-58 18 May 2012. GENERAL: The contractor shall provide milk and dairy products to the CW Bill Young VAHCS located at 10000 Bay Pines Blvd, St. Petersburg Florida 33744. The VA intends to award a firm, fixed price contract for the provision of milk and dairy products. The quantities listed in the attached RFQ are estimated quantities for the Nutrition & Food Services. Contract will be for a base and one option year estimated to begin October 1, 2015 DESCRIPTION/SPECIFICAT1ONS/WORK STATEMENT Delivery of Milk and Dairy Products SCOPE OF WORK: A. General: The scope of this requirement involves providing milk and dairy products, which conform to the specifications outlined and referenced herein, to the CW Bill Young VAHCS Nutrition & Food Service. B. Period of Performance: The period of performance for the provision of milk and dairy products shall be a 12-month base year period from 01 October 2015 - 30 September 2016 plus one (1) 12 month option year.
