SOURCES SOUGHT FOR IMMUNOHISTOCHEMISTRY (IHC) COST PER TEST INSTRUMENT PATHOLOGY AND LABORATORY MEDICINE SERVICE VAMC, WEST PALM BEACH, FL The VA Medical Center at West Palm Beach Florida is in search of a vendor to provide the below Cost Per Test(CPT) type contract/order. Please respond with a capabilities statement that matches the below SOW. Include the type business such as e.g. Small, Woman, Large, Veteran etc. and whether you have a Federal Supply Schedule, VA NAC contract/BPA or open market etc. In addition, if you are a small business please remember one must offer a product/service of their own or of a small business in order to not incur the non-manufacturer rule. The VAMC is not interested in purchasing the equipment and reagents separately or services on the equipment; just CPT type contract. Please respond to [email protected] and [email protected] Purpose: The purpose of this contract request is to procure an immunohistochemical (IHC) staining instrument on a cost per slide basis for the VAMC West Palm Beach Pathology Laboratory. The contractor shall provide, maintain, and retain ownership of all equipment supplied as part of this contract. The estimated workload is 6,000 slides per year for the stains and quantities listed below. The contractor is expected to meet our current staining needs and also be able to expand the stain menu to meet changes in departmental needs. The period of performance is one base year plus four one year option periods plus an additional 6 months for future appropriation problems. Statement of Work: A. Common consumables should be easily replaced without the use of tools. Contractor shall submit documentation of actual description of all tasks and time needed, including both hands on and total time, to perform user-performed maintenance. Must list daily, weekly, monthly, quarterly, 6-month, and annual maintenance tasks. B. Training " Contractor to provide off-site training classes (to include operation, user-performed maintenance, troubleshooting, and operation of software) for a minimum of two (2) VA operators. Training shall be provided by individual(s) qualified to give instruction in the proper operation of the instrument. Additional contractor-performed on-site training of all users must occur prior to start of testing. C. Installation/Maintenance/Service/Project Management " Installation must be performed by a competent technical expert " All service, repair, and maintenance (other than user-performed maintenance) will be performed by the contractor with no additional expense to the government (VA). " System space, plumbing, and electrical requirements must not require modifications to existing space " Installation of instrument shall be performed by contractor. Contractor shall provide all parts, labor, supplies, and equipment to install the instrument. Contractor shall complete function checks during installation to ensure proper operation. " All service, repair, and maintenance (other than user-performed maintenance) shall be performed by the contractor at no expense to the government (VA). " Contractor shall provide equitable adjustment via monetary compensation for any test that becomes unavailable due to instrument malfunction or manufacturer's recall of reagent(s), or manufacturer's inability to supply the reagent(s). " Contractor to provide hazardous waste determination with proposal. The Hazardous Waste Determination is a procedure used to determine whether the by-products of the staining processes are characterized as a hazardous waste. " Pricing: Estimated cost per slide must include all reagent costs, including antibodies, consumables that may be needed (ex. Prep kits), and shipping costs. A listing of all consumable items and pricing must be provided with the CPS estimate. Shipping costs will not be charged separately.
