All equipment delivered by the Contractor shall be FDA approved for use, in new condition, and fully operational in accordance with manufacturer literature specifications. A minimum of two (2) copies of any associated operational and/or service manuals shall be provided for all equipment listed below as applicable. The Contractor shall provide all tools and equipment necessary to properly and safely perform the services set forth within this statement of work. This shall include, but is not limited, to all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner. The Contractor shall provide any equipment necessary for proper infection control measures during installation. 2. AREAS TO BE SERVICED All pieces of equipment shall be delivered to 200 Edison Road, Orange, CT 06477 on a scheduled date and time. Pieces shall be installed on the same date as delivery. 3. INSTALLATION The following items require installation to be provided by the contractor: Otoscope Head- Macroview 3.5v Surecolor LED Fober-Optic w/reusable ear specula 2.75mm KleenSpec Disposable Ear Specula, 8500-Pack 4.25mm KleenSpec Disposable Ear Specula, 8500-Pack Coaxial Ophthalmoscope Home Pistol Cabinet w/key lock (1-Door) Installation will occur during normal business hours, Monday-Friday between 7:30am and 5:00pm. Installation shall be completed within 2 business days. Installation dates shall be scheduled with the VA Medical Center. Installation shall be fully completed prior to departure time. Contractor shall remove debris and packing materials from the site. The VA will not provide a dumpster for disposal of debris. Contractor will be responsible for containing any rubbish/waste generated by the work force or material delivery at the end of the instalaltion. Job site shall be neat and organized to prior to daily departure. 4. TRAINING Training is not required for this equipment purchase. The items listed above will be delivered to 200 Edison Road, Orange, CT, 06447. The Government intends to award a Firm-Fixed Priced contract resulting from the solicitation to the responsive offeror whose offer conforms to the solicitation and will be most advantageous to the government in terms of price. The solicitation will be set aside for SDVOSBs who have the capabilities to fulfill this requirement. Solicitation 36C24120Q0555 will be available for download on August 18, 2019.
