This is a sources sought notice 36C24421Q0653; this is neither a solicitation nor a promise of future work. The Lebanon VAMC is seeking SDVOSB/VOSB and any small or large company regardless of Socio-economic category that can provide the requirement as spelled out in the Statement of Need (SON) and list of items below.
The veteran business must be registered in betaSAM and certified in VETBIZ (VIP). The small or large businesses must be registered in betaSAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought:
Company Name, address, Government Business Point of Contact, telephone number and email address.
Company DUNS number, CAGE Code, and Tax ID numbers.
Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have permission to provide and install any systems or software.

New medical equipment will be required, all resellers, dealer, distributors must be an authorized distributor and provide an authorized distributor letter from the Original Equipment Manufacturer (OEM) stating that fact.

Storz Hopkins II Sinus Telescope and FESS Set
Statement of Need
Background:
The Lebanon VA Medical Center (LVAMC) has a requirement for four (4) Karl Storz, Hopkins II Sinus Telescopes (0, 30, 70, and 45 Degree), accessories, and one (1) accompanying FESS (Functional Endoscopic Sinus Surgery) Instrumentation Set and processing container/s.
Scope of Work:
The Karl Storz Hopkins II Sinus Telescopes of variable degrees, as well as the FESS Instrument Set are used to perform various surgical procedures involving the Sinus Cavity. When a patient has difficulty breathing due to reasons such as polyps, bacteria or fungal infection, chronic sinusitis, structural abnormalities, or cancer, surgery may be necessary. Depending on the location of the issue, various degrees of scopes may be needed to locate and have a good visual of the area. The FESS instruments are then used to perform the procedure indicated to remediate the problem.

Additional Information:
The Karl Storz brand of the scopes and FESS Instruments must be purchased due to the Power Sources and Light Sources to attach the telescopes to and pass the instrumentation through are Storz, and to have integration, Storz Brand is necessary.
Delivery:
Delivery shall be within thirty (30) days from the time of award. The Karl Storz Hopkins II Telescopes, and FESS Instrumentation and accessories shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC OR shall be coordinated through Bio-Med, Joanne Hettel, Surgery Operations Manager, and/or the Contracting Officer Representative.
