The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantage business, woman-owned, small business, or large business) relative to NACIS 325412. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. The intent of this Sources Sought Notice is to identify potential offerors capable of providing Yttrium 90 micropspheres for the primary treatment of Hepatocellular Carcinoma (liver cancer) for the John McClellan VA Medical Center, Little Rock, AR. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of supplies. Please include the following: (a) documentation showing conformance with FDA regulations (FDA approval or exemption of radiopharmaceutical solution to treat primary hepatocellular carcinoma) (b) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements by April 3, 2020 to: Department of Veterans Affairs, NCO 16 - ATTN: Tammie Chaney.
