This RFQ for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. Quotes are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95.
The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, Surgical Appliance and Supplies Manufacturing with a small business size standard of 750 employees.
The Chillicothe VA Medical Center, 17273 State Route 104, Chillicothe OH 45601 is seeking to purchase seven (7) motorized stretchers. This combined synopsis/solicitation requires the exact item, no alternatives will be accepted.
All interested companies shall provide quotes for the Stryker s Primeî Series Stretcher with Zoomî Motorized Drive System and associated items to include installation, delivery, shipping and handling.

The stretchers must meet the following specifications:
Motorized Drive System with advanced technology design that helps reduce strenuous pushing, pulling and steering.
Must be equipped with a Recovery Chair Position.
Must be equipped with Zero-gap Glideaway Siderails..
Must have six wheels with the primary steering mechanism functions on two wheels (not one) helping to eliminate sliding when turning or cornering
This procurement is for new items only; no remanufactured or "gray market" items. No remanufactures or gray market items will be acceptable. Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. Vendor shall be an OEM, authorized dealer, authorized distributor, or authorized reseller for the proposed equipment, verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment shall be in accordance with the OEM terms and conditions.
Place of Performance
The Contractor shall deliver the quoted items, FOB Destination. Delivery shall be completed within thirty (30) calendar days after award, to the following address:

Award shall be made to the contractor whose quote offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria: (1) technical capability factor meeting or exceeding the requirement, (2) past performance, and (3) price.
