SOUCES SOUGHT NOTICE
The Department of Veterans Affairs, Network Contracting Office (NCO) 15 is conducting market research to help determine the availability and technical capabilities of qualified vendors who can deliver radiopharmaceuticals to the Wichita VA Medical Center and within the 50-mile radius of this location. Vendor must be able to start performance on October 1, 2015, base period of one year plus four option years.
POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE:
1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e. small/large), and DUNS Number.
2) Statement of Capability that demonstrates ability of providing the radiopharmaceuticals in accordance with the attached Statement of Work (SOW) and past performance in providing this type of service. Include examples of prior completed Government contracts, references, and other related information.
This notice is to assist the NCO 15 in determining sources only. This announcement is not a request for proposals or quotations. The Government is not committed to award a contract pursuant to this announcement. The Government will not pay for any costs incurred in the preparation or submission of information in response to this announcement.

DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK
RADIOPHARMACEUTICALS
VA Wichita, KS
1. General Description:
Radiopharmaceutical preparation of doses, tagging of blood, and testing shall be performed at contractor facility provided by a board-certified Radiopharmacist and delivered to the Robert J. Dole VA Cardiology Center at 5500 E. Kellogg Wichita, KS. Deliveries will be made Monday through Friday from 6:00am to 4:00pm. Contractor shall provide emergency deliveries as requested by the VA Nuclear Medicine Technician. Personnel assigned by the contractor to perform services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Because of the critical need for these supplies in a timely manner, contractors must be within a 50-mile radius of the Wichita, KS VAMC.
2. Clinical Duties:
Contractor's employees will be given access to the "Hot Lab" that houses the Radiopharmaceuticals in the Cardiology Center for deliveries and pick-ups of blood and C-14 breath samples. Delivery personnel will be bonded by the contractor.
3. Administration:
A. Contractor will bill on a monthly basis, in arrears.
B. All services rendered will be itemized by patient name, date of service, radiopharmaceutical provided, and unit cost.
4. Security and confidentiality:
A. Contractor shall be responsible for maintaining Automated Data Processing (ADP) Security.
B. Contractor shall be responsible for safeguarding and maintaining patient information in the strictest confidence in accordance with the Privacy Act, HIPAA and all other applicable state and federal laws.
5. Delivery Orders:
Orders normally will be placed by 3:00pm. Delivery shall be required by 6:00am the following work day. Due to the demands of the Medical Center's care requirement, there shall be no restriction to the number of orders and number of deliveries required per day placed by the VA Nuclear Medicine Technician's.
The delivery order shall specify: patient name, contract item (s) and quantity, time (s) of delivery and applicable VA order number.
Orders can be placed orally by telephone and/or fax Transmission.

" Deliveries will be made Monday through Friday between the hours of 6:00 am and 4:00 pm.
" The service covered by this contact shall be furnished by the contractor as defined herein. The contractor shall not be required, except in case of emergency, to furnish such services on a national holiday or during non-working hours as described below.

" Access to this location during non-work hours shall be obtained by contacting a Nuclear Medicine Technician at 316-651-3653.
" Deliveries will be supervised by the Nuclear Medicine Technician before beginning delivery operations. Contractor will contact the Nuclear Medicine Technician prior to delivery to arrange for delivery supervision.
7. Delivery Commitment:
Time of delivery is mutually agreed upon at the time of receipt of telephone orders.
8. Person Authorized to Place Orders:
Certified Nuclear Medicine Technician's that are employed by the Robert J. Dole VA Medical Center is authorized to place orders under this contract.
9. Quality Control:
A. Calibration time for each requested radiopharmaceutical will be provided by the Nuclear Medicine Technician at the time of order being placed.
B. The quantity of radioactivity for supplied radiopharmaceuticals will not deviate more than +10% or -5% at the time of calibration, from the quantities specified.
C. The expiration date and time shall be indicated and affixed to each product as noted below; in general:
1. Ready-to-use doses: not shorter than six (6) hours.
2. Tc-99m pertechnetate: Not shorter than twelve (12) hours.
3. Kits intended for reconstitution by user: Not shorter than three (3) months from date of delivery.
4. Unused kits having expiration time not less than two (2) weeks will be exchanged by the Contractor at no additional charge for a comparable kit having the three (3) months life span.
D. In the event that a quality control or material defect is suspected and/or detected by any Veterans Affairs Nuclear Medicine Technician, the Contractor will be requested to provide any consultation necessary to alleviate any said suspicion and/or defect by the following:
1. Provide technical expertise in calibration;
2. Provide professional examination of product; submit results and recommendations to the Contacting Officer;
3. Make expedient replacement of any product suspected of being deficient.
E. PROCEDURE for labeling of patient's white blood cells (WBC) with Indium IN-111 Oxine or Tc-Ceretec for re-injecting to the same patient:
1. Telephone requests to the Contractor to arrange for the procedure will take place no later than 3:00 PM for the next workday procedure.
2. Furnishing of the syringe to the Cardiology Center for nuclear medicine services and pick up of patient's blood specimen for labeling by the Contractor will be at 8:00 AM or when so specified by the Nuclear Medicine Technician's.
3. The return by the Contractor of the patient's Indium labeled white blood cells will take place not later than within 5 (five) hours from the time of specimen pick up time.
4. Pertinent documentation confirming that the identification of the labeled blood specimen matches with the blood specimen of the patient's blood specimen submitted for labeling procedure.
F. Contractor shall produce radiopharmaceuticals as described in the Bid/Price.
G. Contractor shall make quality control records available to the VA Medical Center upon written request from the Contracting Officer.

Contractor must be able to provide the technetium from HEU (highly enriched uranium) to LEU (low enriched uranium) once the Department of Veterans Affairs implements the transition.
