This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA249-16-Q-0648 and the solicitation is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-86-1, March 16, 2016. (iv) This requirement is unrestricted. (v) CLIN0001 - Twelve (12) Boxes of Ten (10) each Vascular Closure Devices, Collagen Plug (Brand Name or Equal to Angio-Seal). (vi) See the attached 1449 CLINS for more information of the description of CLIN0001 above. (vii) The date of delivery is ARO+10. The place of delivery is 1030 Jefferson Avenue Attn: 111, Tiensha Salter, Memphis, Tennessee 38104. FOB is Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. (ix) The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Acceptability of equal to brand name (Angio-Seal) of the item offered to meet the Government requirement; (ii) Price; (iii) Past performance (see Far 15.304); Technical and past performances when combined are of equal importance to price. (x) The provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, shall be on file at www.sam.gov at the time of offer.
