SOURCES SOUGHT ANNOUNCEMENT: The Lebanon VA Medical Center has a requirement for radiopharmaceuticals, kits, and associated materials. Orders will be placed daily for next day, same day, or weekend/holiday delivery. Estimated period of performance is base year plus 4 option years. Estimated date of award is: January 1, 2013. This is a SOURCES SOUGHT NOTICE only. Responses to this notice will be used for information and planning purposes. No proposals are being requested or accepted with this notice. This notice is to obtain possible sources that have the capacity and capabilities to include necessary equipment, personnel, licenses, insurance and qualifications to furnish radiopharmaceuticals. The North American Industry Classification System Code (NAICS) is 325412 and employee size standard of 750 employees will apply to this solicitation. The solicitation package will be available approximately on or about June 29, 2012. Responses to this notice should include the following: 1. Offeror's company name, DUNS, address, point of contact, title, phone number and email address. 2. Offeror's capability to meet this requirement and any pertinent information which demonstrates the firm's ability to meet the above requirements. 3. Offeror's must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals, must perform all quality control procedures as required by Federal and State regulatory agencies, and must meet all Nuclear Regulatory Commission, Department of Transportation, FDA, OSHA and all other agency rules and regulations (Federal and State). 4.
