COMBINED SYNOPSIS/SOLICITATION NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016. Solicitation 36C26218Q0517 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This procurement will be issued as a Veteran Owned Small Business Set-Aside. NAICS Code is 339112 and the Small Business Administration s (SBA) size standard 750 Employees. The Government is seeking a contract to acquire optimal vendor to provide stock supplies for CPAP, masks, AirCurve 10 V Auto with HumidAir, AirCuNe 10 ASV with HumidAir and ClimateLine Air, AirCuNe 10 ST with HumidAir, Mask, Swift LT Nasal Pillows Mask System, Mask, CPAP, Mask, Mirage Quattro Full Face with Headgear - Medium, Mask, Mirage Quattro Full Face with Headgear -Large, and other items listed below. This solicitation is for brand name only and offerors shall be an authorized distributor by the manufacture and must be able to offer full manufacture warranties. Place of Performance: VA Loma Linda Healthcare System (VALLHS), 11201 Benton St. Loma Linda CA. 92357 Period of Performance: Within 10-20 days of placing order or sooner if available.
