The Department of Veterans Affairs VISN 23 intends to issue a solicitation for Prescription Eyeglasses under Request for Quotation (RFQ) 618-197-04 for eligible veterans located in the jurisdiction of VISN 23 which includes the states of North Dakota, South Dakota, Minnesota, Iowa, Nebraska and selected counties in the states of Wisconsin and Wyoming. Delivery must be in five days or less. The successful contractor must have fax capability. Period of performance is November 1, 2004 or date of award through September 30, 2005 with two (2) one-year renewal options, subject to the availability of funds. This procurement is a Competitive Hubzone small business set-aside based on NAICS 339115 Ophthalmic Goods Manufacturing, size standard is 500 employees.
