VA VISN 22 Network Business Center intends to negotiate a commercial (FAR Par 12) sole source procurement with American Medical Depot, under the authority of FAR 6.302-1. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24. Only one responsible source and no other supplies or services will satisfy agency requirements. A justification for other than full and open competition has been prepared in support of this acquisition. This procurement is to obtain One (1) each, Part number 0637-602-01, Opal System, HS English Opal for the Veterans Affairs Loma Linda Healthcare System located at 11201 Benton Street, Loma Linda, CA 92357. Contractor shall provide the Opal Photoactivator Laser System for use with Visudyne Therapy in the treatment of AMD with predominantly classic subfoveal neovascularization (CNV) utilizes a diode laser producing a treatment beam of 689nm wavelength. The Opal Photoactivator's modular design allows easy configuration to fit most diagnostic slit lamp platforms. This feature is exclusive to Lumenis. Exposure time is determined by intensity, fluence, spot size and contact lenses magnification (typically 83 seconds). Exclusive at the link power feedback, variable spot size 400-800mm, footswitch, dual mode (continuous and staged). One pair of safety spectacles. Operators manual, Laser safety sign, two year warranty on parts and labor. Electrical requirements: 100-230C AV, 50/60HZ, 110V, complete installation by a Lumenis trained service engineer is shall be included.
