The US Army Medical Command Southeast Region has a requirement for 1 each Medtronic, Inc. Spinal Cord Stimulator Implant System with vendor representative in attendance during surgical procedure for patient specific surgery. The Government intends to award a sole source contract for this requirement pursuant to 10 U.S.C. 2304(c)(1) and FAR 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements. The suggested contractor is: Jensen Industries Inc., dba Jensen Dental, 50 Stillman Road, North Haven, CT 06473-1622.

6 each spool of 615 Solder, must be compatible with Creation Porcelains which are currently being used at the facility.

3 spools PWS solder 5 dwt with the following alloy content: 71.5% gold, 15% palladium, 10% silver and 3.5% zinc.

17 ounces JCB Gold Alloy type three, high Nobel, yellow, with the following alloy content: 62% gold, 3% palladium, 25% silver and 9% cooper and less than 1% zinc, indium and iridium, must be compatible with Creation Porcelains which are currently being used at the facility.

25 ounces Superior Plus Dental Alloy, Nobel, white, 2% gold, 62.5: palladium and 22% silver. Must have high melting range, cast well, divest easily and be resistant to hot tears.

This IS NOT a request for proposal and no solicitation will be issued. However, companies that can provide an equal product MUST SUBMIT THEIR TECHNICAL CAPABILITIES TO INCLUDE TECHNICAL SPECIFICATIONS OF THEIR PRODUCT, PAST PERFORMANCE INFORMATION, PRICE, DELIVERY TIME AND TECHNICAL/MEDICAL SUPPORT within seven (7) calendar days from the date of publication of this notice. This information will be reviewed by the Government technical representatives from Eisenhower Army Medical Center's U.S. Army Dental Command for consideration for future requirements. This procurement will be prepared in accordance with FAR Part 13, using Simplified Acquisition Procedures.
