(i) This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA250-17-Q-0964 and is issued as a Request for Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-95 (Effective January 13, 2017). (iv) This combined synopsis solicitation is Set-Aside for Service Disabled Veteran Owned Small Business (SDVOSB). The North American Industry Classification System (NAICS) code is 333314, and the size standard is 500 employees. (vii) Contractor shall deliver all items 30 days ARO (After Receipt of Order), FOB Destination to the Saint Joseph County VA Clinic, 1540 Trinity Place, Mishawaka, IN 46545.
