The VA Pittsburgh Healthcare System intends to negotiate on a sole source basis with St Jude Medical, 14901 Deveau Place, Minnesota, MN 55345-2126, to purchase catheters and related accessories: 1) introducer, hemostasis 8.5FR, sro, curve (6 each);2) agilis nxt steerable introducer, small curl, 8.5 french (10 each); 3) catheter, electrophysiology, duo-decapolar, French, multi electrode spacing, electrode tip 2 mm, super large (10 each); 4) catheter - electrophysiology - 6 fr-decapolar (4 each); 5) French josephson supreme quad ep catheter 2-5-2- spacing (5 each); 6) needle, transseptal, brk-1 series, 98 cm, 18 gauge (5 each) 7) introducer, catheter - 8.5 fr sl1, transseptal (6 each); 8) French fast - cath introducer, RAMP curve with tip marker (2 each); 9) universal input module (1 each). The listed cables and accessories are required for the existing catheters. Items are required for the Ensite cardiac mapping system which is used for cardiac mapping/ablation procedures used in the EP Lab. This Notice of Intent is NOT a request for competitive proposals. Parties may identify their interest, accompanied by supporting literature, to the Contracting Officer no later than Wednesday, December 24, 2008, 4:30 p.m. Responses should be sent to [email protected]. No telephone responses will be accepted. It is the belief of the Government that there is currently only one responsible source capable of providing this equipment to satisfy the agency requirement. A determination by the Government not to compete this proposed procurement based on responses to this notice is solely within the discretion of the Government. If no responses are received, VA Pittsburgh Healthcare System will proceed with the sole source procurement from St. Jude Medical.
