Solicitation Number: VA-256-12-Q-1749 Notice Type: Intent to Sole Source The Department of Veterans' Affairs VISN 16 Network Contracting Activity (NCA) intends to award a purchase order on a noncompetitive basis to Surgical Advantage, LLC for the acquisition of Anspach/Synthes EMAX 2 Plus System and peripheral equipment for the VA Medical Center 921 NE 13th Street, Oklahoma City, OK 73104 The award of this sole source contract will be made to Surgical Advantage, LLC, P.O. Box 35565, Tulsa, OK 74153. The statutory authority permitting other than full and open competition is implemented by FAR 6.302-1 (only one responsible source and no other supplies or services will satisfy agency requirements). This solicitation will result in a firm fixed price contract for purchase of the above listed equipment. This requirement will be a small business set aside. The intended procurement will be classified under North America Industry Classification System (NAICS) 423450 with a Small Business Size Standard of 100 employees. A determination by the Government not to compete this proposed contract will be based upon responses to this notice is solely within the discretion of the Government. This Intent to Sole Source neither constitutes a Request for Proposal, nor does it restrict the Government to a particular acquisition approach. No solicitation document is available and telephone requests will not be honored. If however, a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service, to the Contracting Officer within 3 days after publication of this notice. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirement.
