SOUCES SOUGHT NOTICE
The Department of Veterans Affairs, Network Contracting Office (NCO) 15 is conducting market research to help determine the availability and technical capabilities of qualified vendors who can deliver radiopharmaceuticals to the Eastern Kansas VA Medical Centers (Leavenworth and Topeka) and within the 65-mile radius of these two locations: Dwight D. Eisenhower VA Medical Center 4101 South 4th Street Leavenworth, KS 66048 and Colmery O-Neil VA Medical Center at 2200 Gage Blvd Topeka, KS 66622. Vendor must be able to start performance on April 1, 2016 with an anticipated base period of one year plus four option years firm fixed price contract.
POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE:
1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e. small/large), and DUNS Number.
2) Statement of Capability that demonstrates ability of providing the radiopharmaceuticals in accordance with the attached Statement of Requirement (SOR) and past performance in providing this type of service. Include examples of prior completed Government contracts, references, and other related information.
This notice is to assist the NCO 15 in determining sources only. This announcement is not a request for proposals or quotations. The Government is not committed to award a contract pursuant to this announcement. The Government will not pay for any costs incurred in the preparation or submission of information in response to this announcement.

STATEMENT OF REQUIREMENT
Radiopharmaceuticals
VA Eastern Kansas
1. General Description:
Contractor agrees to provide all needed radiopharmaceuticals as identified in the Schedule of Supplies and Prices to Nuclear Medicine Departments at Eastern Kansas Healthcare System Dwight D. Eisenhower VA Medical Center at 4101 South 4th Street Leavenworth, Ks. 66048 and Colmery O-Neil VA Medical Center at 2200 Gage Blvd., Topeka, KS 66622. Deliveries will be made Monday through Friday from 7:30 am to 3:00 pm. Contractor shall provide emergency deliveries as requested by the VA Nuclear Medicine Departments. Personnel assigned by the contractor to perform services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia.
2. Duties:
a. Contractor must be within a 65 mile radius of the Topeka or Leavenworth, KS VA Medical Centers.
b. Contractor shall hold a current unrevoked US Nuclear Regulatory Commission (USNRC) and/or Federal Drug and Administration (FDA) license to produce and distribute radiopharmaceuticals for human administration, and a license to receive and dispose of radioactive waste.
c. Personnel assigned by the Contractor to perform services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia.
d. Contractor shall immediately notify the EK VAMC, upon any action, be from the USNRC, agreement state and/or FDA to suspend or modify the Contractor or supplier.
e. Contractor must be able to provide the technetium from HEU (highly enriched uranium) to LEU (low enriched uranium) once the Department of Veterans Affairs implements the transition.
3. Administration:
a. Contractor will bill on a monthly basis. Obligation/PO number must be listed on the invoice.
b. All services rendered will be itemized by date of service, radiopharmaceutical provided, and unit cost.
c. Orders can be placed orally by telephone and/or fax transmission.
4. Security and Confidentiality:
a. Contractor is responsible for maintaining Automated Data Processing (ADP) security.
b. Contractor is responsible for maintaining patient information in the strictest confidence in accordance with the Privacy Act.
c. Delivery personnel will be bonded by the Contractor.
5. Work Hours:
a. Regular Work Hours: Monday through Friday, 7:30 am to 4:30 pm.
b. Holidays: New Year's Day, Martin Luther King's Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving, and Christmas, and any other day specifically declared by the President of the United States to be a national holiday.
6. Delivery:
a. Orders normally will be placed by 4:00 pm and delivery shall be required by 7:30 am the following work day. Due to the demands of the Medical Center care requirements, there shall be no restriction to the number of orders which may be placed by VA, nor the number of deliveries required per day. In the event that the requirements may be determined in advance, a longer delivery time will be scheduled accordingly.
b. The delivery order shall specify: contract item(s) and quantity, time(s) of delivery, and applicable VA order number.
c. Deliveries will incorporate sufficient lead or other high-density metal shielding to comply with all applicable federal and/or state standards relating to shipment of radioactive materials.
d. Deliveries will be supervised by the COR or their designee before beginning delivery operations. Contractor will contact the COR prior to delivery to arrange for delivery supervision.
e. If shipments are contracted to be delivered specifically on Mondays, but Monday is a holiday, then the shipment will take place the next regular work day.
f. Time of delivery specified or mutually agreed to at the time of receipt of telephone or fax order shall become mandatory. Failure to perform in accordance with the time of delivery may be grounds for termination of contract.
7. Person Authorized to Place Orders:
In addition to the Contracting Officer, the COR is authorized to place orders under this contract, as well as any Certified Nuclear Medicine Technologist employed by the Eastern Kansas VA Medical Center.
