YWCA of Greater Los Angeles which operates the Los Angeles Job Corps Center (LAJCC) under contract with the U.S. Department of Labor, Employment and Training Administration is requesting bids to provide dental services under contract for youth between sixteen and twenty four years old. The dental services will be provided in our facility located at 1106 So. Broadway Ave, Los Angeles, CA 90015. Dentist is required to: 1) Work 40 hours every two weeks consisting of Tuesday and Thursday and every other Monday. 2) Must be licensed in the State of California. 3) Must provide Professional Liability Insurance and Controlled Substance Registration Certificate. The dental services will include general practice: 1) Preventive dental health education. 2) 24 hour emergency on call coverage including weekends. 3) Oral examination, to include four bite-wing X-rays. 4) Restorative and prophylactic treatment. 5) A diagnosis and treatment plan for each student within two weeks after completion of routine dental examination, including a listing of the teeth involved and specific services to be rendered. 6) The dental services will be furnished for one year starting December 1, 2003 through November 30, 2004 with the option to extend the contract for one additional year as permitted by the Department of Labor, Office of Job Corps.
