This is a combined synopsis/solicitation, Request for Quotation 0401-4-0015, for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented without additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 19. This is 100 percent small business set-aside. The NAIC code is 488390 the size standard for this code is $6M. Furnishing of a dock, which shall meet the following specifications: DOCK 8’ x 60’ Tee on an 8’ x 40’ primary base with two 24’ x 26’ wet berth slips. 4’ x 26’ arm separating Tee and primary base. FRAME 12” hot-dipped galvanized truss frame. DECK Trex material or equivalent, on 100% of dock. FLOTATION Polyethylene or equivalent, encased floats. FENDER Covers entire perimeter of dock. ACCESSORIES 4’ x 16’ aluminum bridge with hinge plate and 5th wheel connection at bridge and dock. Deck cleats spaced the entire perimeter of dock. Connection piano hinges that allow for fluctuations of water and 3 to 4 foot waves. ANCHORAGE Winch with holding capacity 10,000 lbs and lifting capacity 3,000 lbs with adjustable handle. Also with anchor tubes. DELIVERY Delivery time is July 15, 2004. Delivery instructions: Shipping and off loading of said items at the Lucerne Warehouse approx 5 miles from Manlia, UT 84046. The provisions of 52.212-1, Instructions to Offerors-Commercial; 52,212-2 Evaluation-Commercial Items, 52,212-4, Contract Terms and Conditions-Commercial Items apply to the RFQ. Evaluation Criteria to be used in FAR 52,212-2 Evaluation-Commercial items are (1) dock specifications (2) past performance (including delivery time) and (3) price. Offerors must include descriptive literature on dock offered, warranty period, price, references and phone numbers, 52.212-3, Offeror Representations and Certifications filled out.
