Page 3 of 3 Notice of Intent to Sole Source The Erie VA Medical Center, 135 E. 38th St., Erie, Pennsylvania has a requirement for brand name Tyvaso 0.6 MG/ML REFILL KIT. VA has only located one vendor capable of providing these reagents and intends to issue a sole source contract to that vendor. This notice is being posted so that any unknown vendors who are capable of providing these items may come forward and express their interest and capability in providing these specific reagents to VA. If your company/organization is capable of providing the required items, please provide your entities capability statement to Allan Tabliago, Contracting Officer by 3:00 p.m. ET, Friday, September 10, 2021 via email to [email protected]. In addition to your capability statement please also provide any applicable distribution agreements regarding these reagents, the size and socioeconomic status of your company, and any other information pertinent to this requirement. Reference Procurement ID No. 36C24422Q0005 in the subject of the email. ***This is not a request for quote, no quotes should be sent in response to this notice. Tyvaso 0.6 MG/ML REFILL KIT 1. Background: Service: Pharmacy Requestor: Medical Doctor The Erie, PA VAMC has a veteran that needs an expensive prescription drug (Tyvaso). This prescription drug costs $11,661.75 per month and exceeds purchase card thresholds. Additionally, since this is a reoccurring procurement a contract needs to be established. This prescription drug is to be delivered monthly to the veteran s home. The pharmacy department within the Erie, PA VAMC is funding this contract from October 1, 2021 through March 31, 2022. These six (6) months of this prescription drug procurement will be placed against the current NAC Contract No. 36F79720D0104. Performance Monitoring: Pharmacy department shall monitor monthly by proof of delivery that prescription has been received by veteran. Privacy and Security Requirements: The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall require intermittent access only and will be escorted by VA employees while at VA Facilities.
