This is a combined synopsis/solicitation, Request for Quotation 0401-5-0005, for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented without additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 19. This is 100 percent small business set-aside. The NAIC code is 488390 the size standard for this code is $6M. Furnishing of two docks, which shall meet the following specifications: DOCK 8’ x 60’ Tee with an 8’ x 40’ primary base. 4’ x 16’ aluminum bridge with hinge plate and wheels on shore end. Fifth wheel/rotating dock bridge connection with hinge plate. FRAME 12” hot-dipped galvanized truss frame. DECK Trex material or equivalent, on 100% of dock. FLOTATION Polyethylene or equivalent, encased floats. ACCESSORIES 10 Deck cleats spaced the entire perimeter of dock. Connection piano hinges that allow for fluctuations of water and 3 to 4 foot waves. ANCHORAGE Two Winchs with holding capacity 10,000 lbs and lifting capacity 3,000 lbs with adjustable handle and anchoring accessories. DELIVERY Delivery time is August 01, 2005. Delivery instructions: Shipping and off loading of first dock at the Sheep Creek Boat Ramp located 10 miles South of Manila, UT 84056. Second dock at the Mustang Ridge Boat Ramp located 2 Miles North of Dutch John, UT 84023. The provisions of 52.212-1, Instructions to Offerors-Commercial; 52,212-2 Evaluation-Commercial Items, 52,212-4, Contract Terms and Conditions-Commercial Items apply to the RFQ. Evaluation Criteria to be used in FAR 52,212-2 Evaluation-Commercial items are (1) dock specifications (2) past performance (including delivery time) and (3) price. Offerors must include descriptive literature on dock offered, warranty period, price, references and phone numbers, 52.212-3, Offeror Representations and Certifications filled out. Vendors must register in the Central Contractors Registration (CCR) in order to be awarded a contract. See http://www.ccr.gov/handbook.cfm for a template of information necessary for registration. Quotes must be received by 4:30 PM June 01, 2005, please include drawings and specifications of quoted dock. BUSINESS SIZE IS AMENDED TO UN-RESTRICTED.
