VA NORTHERN INDIANA HEALTHCARE SYSTEM-FORT WAYNE CAMPUS PROJECT: 610A4-25-503 Replace Containment Sump Statement of Work: Contractor will provide all labor, equipment, materials, and supervision necessary to replace the containment sump, spill container, and associated valves, piping, and other material outside Building 16. This will include excavation, removing and disposing of the containment sump, spill container and all associated material, installation of new containment sump, spill container and all associated material. All work to be completed at the VA- Northern Indiana Health Care Systems Fort Wayne Campus, 2121 Lake Avenue. Fort Wayne, IN 46805-5100. Work to be completed within 90 calendar days from notice to proceed. Line items include but are not limited to the following: Existing concrete above existing underground storage tank to be broken and disposed of. Excavate to tank top keeping existing backfill aggregate on site for reuse. Existing spill container, fill riser pipe and drop tube to be disconnected, removed, and disposed of. Existing tank top containment sump to be disconnected, removed, and disposed of. Supply and install one (1) new fill riser pipe, spill container and overfill prevention drop tube. Supply and install one (1) new fiberglass tank top containment sump along with supplied entry fittings and mounting hardware. Connect to existing product piping. Excavation to be backfilled. Concrete to be poured and finished. New containment sump and spill container to be hydrostatically tested and documentation provided for records. Additional information: Installation shall comply with TIL Section 23.10.00 and industry standards. Requirements for manual tank gauging in the event that the automatic tank gauging system is disconnected. UST tank installation projects require a certified tank installer, that installer shall provide a certification of compliance on the UST Notification Form in accordance with 40 CFR 280.22. For downtime less than 24 hours, readings must be conducted and recorded every hour until the automatic tan k system is back online. For downtime of more than 24 hours to 1 week, readings must be conducted and recorded every 6 hours. For downtime that is expected to last more than 1 week, reading must be conducted and recorded once a week. Contractor shall fill out and certify IDEM s UST Notification Form to communicate to the state of the completed updates. This is not a solicitation. The government intends to issue a solicitation for this requirement on or about May 19, 2025. At this time, the contracting office is not able to answer questions pertaining to this notice. Should the Government post a solicitation, additional information will be provided, including specifications, a site visit, and a request for information process.
