Issuing Procurement Unit State of Utah Division of Purchasing Conducting Procurement Unit State of Utah Division of Purchasing REQUEST FOR STATEMENT OF QUALIFICATIONS Veterinary Services For Livestock Auctions And Brucellosis Surveillance SOLICITATION #GJ20-12 This Request for Statement of Qualifications (“RFSQ”) having been determined to be the appropriate procurement method to provide the best value to the Conducting Procurement Unit, is issued in accordance with State of Utah Procurement Code, Utah Code Annotated (UCA) Chapter 63G-6a, and applicable Rules found in the Utah Administrative Code (UAC). If any provision of this RFSQ conflicts with the UCA or UAC, the UCA or UAC will take precedence. Purpose of this Solicitation The State of Utah, Division of Purchasing has issued this RFSQ to obtain statements of qualifications from companies interested in providing 1.) Veterinary Brucello Surveillance and 2.) Veterinary Services for Livestock Auctions with the intent to establish an open-ended approved vendor list. Vendors approved during this process will be prequalified for a period of 18 months after which time an approved vendor must resubmit qualifications for evaluation and renewal. The State retains the right to reopen the solicitation to allow additional vendors to submit responses at any time, but no longer than every twelve months, during the term of this open-ended approved vendor list. The RFSQ can be reopened for any reason the State deems appropriate. Closing date and time The closing date and time for this sourcing event is 03/17/2040 03:00 PM Mountain Time. Issuing Procurement Unit, Conducting Procurement Unit, and Solicitation Number The State of Utah Division of Purchasing is the issuing procurement unit and conducting procurement unit for this RFSQ (referred to as “the State”). The reference number for this RFSQ is Solicitation #GJ20-12. This solicitation number must be referred to on all bids, correspondence, and documentation submitted to the State relating to this RFSQ. Additional Information Vendors are prohibited from communications regarding this RFSQ with the conducting procurement unit staff, evaluation committee members, or other associated individuals EXCEPT the State of Utah Division of Purchasing procurement officer overseeing this RFSQ. Wherever in this RFSQ an item is defined by using a trade name, brand name, or a manufacturer and/or model number, it is intended that the words, “or equivalent” apply; and invites the submission of equivalent products by the Vendors. Vendors may be required to submit product samples to assist the chief procurement officer or head of a procurement unit with independent procurement authority in evaluating whether a procurement item meets the specifications and other requirements set forth in the RFSQ. Product samples must be furnished free of charge unless otherwise stated in the RFSQ, and if not destroyed by testing, will upon written request within any deadline stated in the RFSQ, be returned at the Vendor's expense. Samples must be labeled or otherwise identified as specified in the RFSQ by the procurement unit. The State reserves the right to conduct discussions with the Vendors who submit Statements of Qualification determined to be reasonably susceptible of being selected for award. Evaluation of Statements of Qualifications Each Statement of Qualifications received shall be evaluated for responsiveness in as outlined in the Utah Procurement Code. Pursuant to Section 63G-6a-410 of the Utah Procurement Code this RFSQ shall be evaluated by the objective criteria described in this RFSQ. Statements of Qualifications submitted must comply with the prerequisites and questions sections of this RFSQ. Any Statement of Qualification that does not comply with the prerequisite and questions sections will be deemed non-responsive and will be rejected. Any exceptions to the content of this RFSQ, including the prerequisites, must be protested in writing to the Division of Purchasing prior to the closing date. To be responsive and responsible Vendors must review and respond to the following sections of this RFSQ: Prerequisites, Buyer Attachments, and Questions. The Prerequisites Section contains the objective criteria that will be used to evaluate the bids, including the mandatory minimum requirements and general requirements that Vendors must certify that they have read, understand, and agree in order to submit a bid. The Buyer Attachments Section contains the required State of Utah Terms and Conditions of the RFSQ, which are non-negotiable, the General Provisions which apply to this RFSQ, and any additional required forms. The Questions Section contains the questions that Vendors are required to answer in order to submit their Statement of Qualifications. Vendors must review each section carefully.
