Thank you for your interest in providing fundraising services to Plano ISD! We appreciate your support and cooperation as we take steps to ensure good communication and overall efficiency in fundraising operations. Our goal is to better track and evaluate the most successful fundraising opportunities so that our schools and student organizations can maximize their revenues for Plano ISD schools/programs. All companies interested in providing fundraiser services to the District must complete this Fundraiser Application. The District reserves the right to determine if the fundraising services are acceptable. If the fundraising service is deemed unacceptable, the application will be denied. APPLICATION PROCESS AND REQUIRED DOCUMENTS: Complete the Fundraising Application in Ionwave Attach W-9 Attach Proof of Insurance - Add Plano ISD as an additional insurer (please refer to Bid Attribute #4) Conflict of Interest Questionnaire - **ONLY COMPLETE THE CIQ FORM IF THERE IS A DISCLOSURE.** Attach catalogs, brochures, order forms, etc. under the "Response Attachments" tab It is anticipated that the review process may take 2 weeks. However, the process may be delayed due to an increased number of applications or a delay in reference responses. APPROVAL of APPLICATION: Plano ISD anticipates approving multiple vendors on an incremental basis in order to best serve the needs of the district. Plano ISD does not guarantee the volume (quantity or dollar value) to be purchased annually. The actual amount to be purchased depends on needs of the campuses. This application will not be a guarantee of purchase of any goods or services, but will establish you as an Approved Fundraising Vendor with Plano ISD. Approval will be made upon acceptance and review of applications Each applicant will be contacted in writing of their approval No contact with the campuses shall take place until receipt of the approval CARD DENIAL of APPLICATION: Applications may be denied for any of the following reasons, but not limited to: Failure to meet the acceptable requirements set forth in the Evaluation Criteria above Product/service is unsuitable for marketing in the District Insufficient profit potential for Plano ISD TERM: The term of this application shall be one (1) year with four (4) successive one (1) year auto-renewal, based on the long-range needs of the District and mutual consent of both parties not to exceed five (5) years total. Initial Contract Period: July 1, 2021 - June 30, 2022 Contract YR2 (Option): July 1, 2022 - June 30, 2023 Contract YR3 (Option): July 1, 2023 - June 30, 2024 Contract YR4 (Option): July 1, 2024 - June 30, 2025 Contract YR5 (Option): July 1, 2025 - June 30, 2026 Please Note: NEW VENDORS ARE REQUIRED TO REGISTER IN IONWAVE FIRST IN ORDER TO RESPOND TO THIS APPLICATION.
