Description of Services and Deliverables A) Service Location: Department of Rehabilitation 705 Pine Street Red Bluff, CA 96080 B) Commercial Shop: The Contractor's principal point of business or branch office must be within thirty-five (35) miles of the Red Bluff Office. C) Type of Service: The Contractor shall furnish all labor, equipment, cleaning agents, materials and supplies required to provide general office maintenance for approximately 1,148 square feet of office space as located in the scope of work. D) Population This office has 12 Staff and approx. 20 to 30 consumers daily. SERVICES TO BE PROVIDED DAILY: Restrooms/Break rooms a) Pick up paper and empty all waste containers and re-line trash containers. Plastic liners to be furnished by the Contractor. b) Flush and check all toilets for proper flushing operation. Any malfunction shall be immediately reported to the Contract Administrator. c) Clean and maintain in a clean, sanitary and odor-free condition all floors, mirrors, sinks, and sanitize all toilet bowls and urinals. d) Damp mop floors with a disinfectant or germicidal solution. Floors shall be maintained in a clean state, free of build-up, dirt and black markings. e) Furnish and replenish all rest room supplies (including soap, towels, seat covers, toilet tissue, sanitary napkins and tampons). f) Spot clean the walls, partitions, doors, doorframes, and light fixtures. Offices and Corridors a) Empty and clean all trash containers and ashtrays and dispose of all trash and rubbish. Reline trash containers. Plastic liners to be provided by the Contractor. b) Sweep or dust mop all hard surface floors and vacuum all carpeted areas, including halls and any stairways. c) Remove spots and/or spills from the carpets, rugs, floors and any stairways. d) Spot clean the walls, partitions, doors, doorframes and light fixtures. e) Dust the tops of all furniture, counters, cabinets and window sills (which are free of interfering objects). f) Remove finger marks and smudges from all glass entrance doors. g) Clean and polish drinking fountains to eliminate stains and mineral deposits. SERVICES TO BE PROVIDED WEEKLY Restrooms/Break rooms a) Damp mop all hard surface floors. b) Clean walls. c) Clean vents and grills. Offices and Corridors a) Damp mop all hard surface floors. b) Treat stainless steel fountains and sinks to eliminate stains and mineral deposits. SERVICES TO BE PROVIDED MONTHLY a) Dust and/or vacuum air vents b) Dust Venetian blinds. c) Buff and polish all hard surface floors. SERVICES TO BE PROVIDED SEMI-ANNUALLY a) Strip and apply a new coat of non-slip wax to all hard surface floors and buff as necessary. b) Wash all interior and exterior windows, Venetian blinds, light fixtures, walls and painted surfaces. c) Steam clean carpets and/or rugs (using no soaps or detergents) to remove all stains and spots and treat for static electricity control. CUSTODIAN SUPPLIES (including, but not limited to) the following: a) Custodian cleaning supplies b) Soap c) Seat covers d) Toilet tissue e) Sanitary napkins f) Tampons g) Paper towels* h) Plastic liners for trash cans* i) Light bulbs and fluorescent tubes j) Automatic air freshener and refills *Items to be replaced in employee break room as well as restrooms. Contractor is required to replace inoperative light bulbs and fluorescent tubes throughout office area of consistent color and install, furnish, and maintain a battery-operated automatic air freshener for each restroom. D) Call Back Services: Should the Contractor fail to provide any of the specified services in the prescribed manner, the State may call the Contractor to provide services within four hours, unless otherwise specified, at no additional cost to the State. E) Emergency Services: The Contractor shall make emergency calls during non-business hours, if necessary. If the need for repair was created by the State, the State agrees to reimburse the Contractor for each emergency call at the employee¿s hourly rate at time-and-a-half and not to exceed $25.00 per hour. F) Hours: Services must be provided Monday through Friday, between the hours of 5:00 p.m. and 6:00 a.m., holidays excluded. A list of official State holidays is available upon request from the Department of Rehabilitation. [email protected] H) State Property: 1) It shall be the contractor's responsibility to make all necessary arrangements with office/building manager and local traffic authorities for use of elevators, loading docks, adjacent streets, sidewalks, and/or malls. 2) Any damages by the contractor to portions of buildings, premises, equipment, furniture, material or other property for which the State is responsible will be repaired or items will be replaced by contractor to the satisfaction of the State with no expense to the State. I) Recycling Product Content: a) This Contract requires the use of custodian supplies containing recycled paper products only. b) The Contractor must agree to certify in writing, upon completion of performance under the Agreement, the minimum percentage, or the exact percentage of post-consumer and secondary materials provided, or used in the services provided under the Agreement.
